If you need to hire an Interpreter, please gather the following information:
Date and Time you need the Interpreter.
Place you want the Interpreter to go to.
Name of the Deaf person and any requests they may have. (Certain type of Interpreter, name of a preferred Interpreter, preference of male or female Interpreter, etc....)
Billing Information.
Reason you need the Interpreter (Meeting, medical procedure, court hearing, etc....) and the length of time you expect to need the Interpreter.
A Contact Person on site where you want the Interpreter to report and their phone number should any problems occur such as not being able to get past security.
If your request is for an Interpreter in less than 24 hours, please call our 24 Hour Emergency Services Division at 810-459-3967 for immediate assistance.
If you request is after noon on Friday for during the weekend, Monday, or just before a holiday, please call the above 24 Hour number to ensure proper services.
If you are requesting an Interpreter with more than 24 hour notice and not within the stipulations above, you may download and print the form below, SLS Interpreter Request Form, fill it out and fax it to 586-771-2042, email it to Interpreter@SignLanguageServicesofMichigan.com or fill in the form below with the information requested above in the Assignment Information section, call us at 586-778-4188 or mail the form to Sign Language Services of Michigan, LLC, 31507 Harper Ave., St. Clair Shores, MI 48082
